The County Durham Care Partnership has been announced as a finalist in the 2021 HSJ Awards recognising outstanding contribution to health and social care.
The Partnership brings together NHS organisations such as County Durham and Darlington NHS Foundation Trust, Tees, Esk and Wear Valleys NHS Foundation Trust, the County Durham Clinical Commissioning Group, Durham County Council and other health and care providers to work in true collaboration to deliver improved health outcomes for local people.
Through an imaginative and innovative programme of integration, the County Durham Care Partnership has improved outcomes for service users, including improved experience of care and support for service users, delivering care closer to home, less duplication and an emphasis on prevention and early help.
Live judging will take place on 28 September with the winners announced at an awards ceremony on 18 November.
