We are a group of 63 member general practices which have come together to commission (or buy) local health services for people who live within County Durham. Our practices have worked together for many years in locality areas which as a Clinical Commissioning Group we continue to support today.
Our six geographical areas are:

- Sedgefield

Each of our member practices is also part of a Primary Care Networks (PCN). PCNs are smaller groups of general practices working closely together with other primary and community care staff and health and care organisations to provide integrated services to their local populations. These networks have been established to help deliver the NHS Long Term Plan. County Durham PCNs were launched on 1 July 2019 and more detail is available on our Primary Care Networks page.
